All questionnaires must be completed 7 days before your appointment. These forms take several days to complete. Thus, please make your appointment at least 10-14 days in advance to give you plenty of time to fill out the forms.

There is a fee for cancellation within 48 hours of an appointment.

All questionnaires must be submitted 7 days before an initial appointment or the appointment will be cancelled.

All payments are due at the time of consultation.
