APPOINTMENT RESERVATION POLICIES:

There is a 50% deposit for a new patient appointment, which will be credited towards the consultation, once completed. This will be refunded only if the appointment is cancelled with more than 7 days notice.

Initial consultation appointments should be made approximately 14 days in advance, to allow plenty of time to complete the in-depth medical and lifestyle questionnaires, including a 3-day diet diary. 

The initial medical questionnaires must be completed at least 7 days before the appointment. If the initial medical questionnaires are not completed by 7 days before an appointment, the appointment will be rescheduled and cancellation fees will be applied.

All payments for the consultations, laboratory test kits, and supplements provided in-office, are due at the time of service.

A credit card is required on file via the electronic medical records secure site.

No medical insurance is accepted, but a "superbill" will be provided that can be submitted to insurance for possible reimbursement. Note that many health insurance policies do not cover functional medicine or diet and lifestyle counseling. 

CANCELLATION POLICY:

A minimum of 7 days’ notice is required for cancellations of new patient visits, and 3 days’ notice for existing patients. If cancellation is within these time limits of the appointment time, the deposit will be non-refundable. The appointment may be rescheduled however. Additionally, there will be a 100% office visit charge for “no-shows” or late cancellations. Accommodations are made for emergency situations. 

initial consultation and office visits:

Office visits and telemedicine consultations are billed at $750 per hour. The first office visit is 3-5 hours and costs $1500. The lab review follow-up visit is usually 2-3 hours and costs $1500. This may be broken up into shorter visits. All other follow-up visits and phone calls are billed at $750 per hour. 

BILLING:

We accept cash, checks, credit cards, FSA and HAS cards. We do not accept insurance, nor do we bill insurance or prepare HCFA forms for you. However, we will furnish you with an itemized bill for self-submission. There is no refund for services provided.

For Medicare patients: We do not accept Medicare in our office. All services, including labs and mail away specialty lab tests are self-pay. (Sometimes, Medicare will cover some conventional laboratory testing and some specialty testing). You will not be able to submit any bill to Medicare for reimbursement for any services you encounter in our office. Please sign the additional Medicare Private Contract form stating that you understand these rules.

There is a standard $50.00 service fee charged for pursuing prior authorizations from insurance companies. This fee will also be charged for provider time spent on completing simple letters and forms related to work/school/travel. Additional fees may be charged for more complex or time-intensive administrative work.

Because of teaching, traveling and research, Dr. Cantor-Adkins is not available on a 24-hour basis at all times. Therefore, you must have a primary care doctor with whom you can consult in the event of an emergency or urgent problem. If you have a serious health problem that requires immediate attention, you should call your other doctor(s), call 911, or have someone take you to the nearest hospital emergency room. If you notice an adverse effect from one of the components of your health plan, you should discontinue it immediately and email Dr. Cantor-Adkins at stacey@cantoradkinsmd.com or call 917-525-5025.

Please note that Dr. Cantor-Adkins is not a primary provider and does not complete disability or workers’ compensation forms for patients.

SPECIALTY LABORATORY TESTING:

We routinely use specialty testing. These are usually an out-of-pocket expense, although there may be some insurance coverage. We will guide you through identifying the costs associated with your testing.

ANONYMOUS PATIENT INFORMATION FOR EDUCATIONAL PURPOSES

Dr. Cantor-Adkins will ask your permission to use or to describe your anonymous medical history and laboratory data for educational purposes in lectures, blogs, case reports and other publications that are communicated to other professionals, but may include members of the public. This medical history and laboratory data might include photographs and/or images of parts of my body other than your face (nutrition/physical exam findings only). Stacey L. Cantor-Adkins, M.D., PLLC will never publish any information that uses my name or that identifies me as the source of any of the information, data, or images that is publishes. You may choose to opt out of the use of your medical information in this way.
